  • Sedentary personnel likely to have to carry out internal investigations after an accident or incident (Armament, Operations, Technical, Health, Safety and Environment departments),
  • Flying personnel subject to customer Vetting (SIRE, OVID),
  • Elected members of the Social and Economic Committee,
  • Members of the Health, Safety and Working Conditions Committee

PROGRAMME

Half-day 1 : 4 hours

-Expected skills :

Identify the reasons for investigating accidents, incidents and near misses. Identify the main stages of investigation. Identify the most common investigation tools used in industry and the maritime world.

-Contents

Employer obligations (Deming wheel, ISM, ISPS) and maritime industry standards (Vetting), Quantitative and qualitative analysis.

Setting up the investigation team

Collection of initial and in-depth data

Presentation of investigation tools (QQOQCCP / 5W, Cause Tree, 5 Whys, Hishikawa, Bow Tie, tap root)

 

Half-day 2: 4h

-Expected skills :

Identify the structure of an investigation report.

Be able to use investigation tools and carry out an investigation on a simple case.

Analysis, writing and integration, conclusion and report

Case studies and practical exercises
